The Duchess of Cambridge was attending the St Patrick's Day celebrations with Prince William at Mons Barracks in Aldershot, Hampshire when the incident occurred.
The royal couple were seen smiling when one of Middleton's high heels became caught in a drain during the parade.
Prince William held onto her hand as she freed her shoe from the grate, before they met with soldiers from the 1st Battalion Irish Guards.
The Duchess then proceeded to present officers and guardsmen with traditional shamrock sprigs, a role which was previously carried out by the Queen Mother among others.
Prince William and the Duchess of Cambridge were seen enjoying a day out at the Cheltenham Gold Cup on Friday (March 15).
